1. I authorize Ike Gaming Inc., to conduct any necessary and reasonable investigation regarding information on my application and release Ike Gaming Inc., my former employers and any other references from any liability from damage caused by giving and receiving information or opinions as to my employment or character.
2. I understand and agree that any person authorized by management can at any time request that I submit to a search of my person, purse, packages in my possession, or any locker that may be assigned to me. I understand that my refusal to submit to such a search may result in termination of employment.
3. I agree to, if hired, submit to the Ike Gaming Inc. drug testing procedure to determine if I am under any illegal drugs. I understand that failure to submit to drug testing may result in refusal of employment, or if once hired, in my termination. If hired, I agree to, if requested by Ike Gaming Inc., have a medical examination at Ike Gaming Inc.'s expense, to determine my fitness to work. In that event, I authorize any physician to release any information which may be necessary to determinate my ability to perform my assigned duties. I understand that any positive results of the use of illegal drugs may result in refusal of employment, or if employed, termination of employment.
4. If hired, I agree to conform to all of Ike Gaming Inc.'s policies, procedures or practices, and that my employment can be terminated, with or without cause, and with or without notice, at any time, at the option of Ike Gaming Inc. or myself. I understand that my failure to abide, if employed to Ike Gaming Inc.'s policies, procedures, or practices may result in termination of employment.
5. No officer, agent, or employee of Ike Gaming Inc. has promised me employment for any specific amount of time.
6. I understand that this document is an applicant of employment. If I am employed, such employment is for an indefinite amount of the time, and the company can change wages, benefits, conditions, policies and procedures at any time (except as otherwise by law).
7. I understand that as a condition of employment, I must be flexible on my shifts and days off and may be required to work a reasonable amount of overtime. I understand that Ike Gaming Inc., at its discretion may change my shift and days off. I understand that if I fail to report for work as scheduled, my employment may be terminated.
8. I promise to pay Ike Gamign Inc. for any and all items entrusted to me which are not returned in good condition, and for any damages or liability I may cause to Ike Gaming Inc., and I hereby authorize Ike Gaming Inc. to withhold from my final paycheck any amounts thus owed.
9. I authorize any previous employer, law enforcement agency, credit bureaus, now or in the future to release any information about me. Furthermore, I understand that any and all information listed on my application is subject to inquiry, and that any misrepresentation of facts, or any insufficient or unverifiable information listed on my application will be sufficient cause for refusal, or if employed, termination of employment.

The foregoing is an accurate statement of the facts to the best of my knowledge and I understand that any falsification, incomplete information or misrepresentation may be reason to refuse me employment or cause disciplinary action, including termination, if hired and discovered later.

I understand and agree that my employment is at will, that nothing in this application or in any other express or implied agreement shall be deemed to create or become part of any contract of any employment for a specified term between me and El Cortez and that my employment can be terminated at any time by myself or El Cortez for any reason. I understand and agree that any statements to the contrary, whether oral or written, are expressly disavowed and are not to be relied upon by me. I further understand that no representative of El Cortez has any authority to enter into any agreement for employment for any specified period of time or make any agreement contrary to the foregoing.

I give El Cortez the right to secure additional job related information about me. I hereby release El Cortez and its representatives from liability for seeking such information and all other persons, corporations or organizations from furnishing such information.

I understand that I will be required to take a pre-employment drug test, and if hired, I will also be required to undergo both for cause and random drug testing. I further understand, if hired, that I will be terminated if I fail to pass any drug test or refuse to take any drug test.

I realize that the El Cortez operates a casino in which alcoholic beverages are served and in which smoking is permitted.

I agree to abide by all El Cortez rules, policies and procedures. If I am hired, I understand El Cortez may amend these at its discretion and those do not constitute an employment contract. All such rules, policies and procedures are available to employees through the Human Resources Department.

El Cortez does not discriminate in employment and no question on this application is used for purpose of limiting or excluding an applicant's consideration for employment on any basis prohibited by local, state or federal law.
